Medical and Liability Release

By marking the checkbox below on behalf of my child, I assume the risk associated with dance instruction/classes and agree that the Hill Dance Academy Theatre and its respective Board of Directors, faculty, staff, and volunteers shall not be liable in any way for any injuries sustained or loss of property while attending a dance class or any related functions. In the event that I am not present at the class, I hereby grant permission to instructional staff to authorize hospital admission and medical, surgical, and emergency treatment, including blood or blood product transfusions and diagnostic procedures.

Additionally, I grant permission for the administration of anesthesia for the student where medically necessary in case of emergency, accident, and illness and only in the case that the parent or alternate family representative cannot be contacted​.

In Dance training, a faculty member may place their hands on a student to correct the student’s posture, the physical line, the position of the student’s body or any part of it, or to help the student hold a position. Hill Dance Academy Theatre doesn’t tolerate any inappropriate or harmful faculty-student contact. By participating in Hill Dance Academy Theatre activities and programs, I acknowledge that Hill Dance Academy Theatre faculty/guest artists may correct students with physical contact.

Tuition Terms

Tuition is based on the level of your dancer. You can pay tuition in one lump sum before the start of the season or sign up for a tuition plan. Once tuition is paid, it is non-refundable. Once you sign up for a plan, the plan will continue until your tuition is paid in full. Please understand that you are taking the spot of a potential student, and tuition is paid even if your student decides to resign from classes. If your student is expelled from the program or a medical emergency arises, tuition plans will be canceled. A medical professional needs proof of the medical emergency (mental or physical).

Photography and Video Release

Hill Dance Academy Theatre takes photographs and videos of students enrolled in academy classes, students taking master classes, and students attending events and performances while in our facility.  From time to time during the season, students may be interviewed, videotaped and or photographed by representatives of the external news media covering HDAT for public Information.

The photographs and videos taken are all for Hill Dance Academy Theatre promotion, publications, newsletters, flyers, publicity, brochures, website use, social media sites, other electronic communications, or video usage. 

Students will not be paid or compensated by Hill Dance Academy Theatre or any person/entity in any way for use of their image, likeness, or commentary. Authorization grants Hill Dance Academy Theatre rights to use of the material in perpetuity.

All photography/video is property of Hill Dance Academy Theatre and may be used beyond the 2026-2027 season.

Hill Dance Academy Theatre shall have the right to exercise such rights with my appearance, likeness, and form alone, or with other materials, including, but not limited to, text, data, images, photographs, illustrations, animation, graphics, and video or audio segments of any nature. The rights granted to Hill Dance Academy Theatre hereunder are world-wide, irrevocable, perpetual, and assignable. I hereby waive all rights and release Hill Dance Academy Theatre and its Board of Directors, officers, employees, agents, and independent contractors from, and shall neither sue nor bring any proceeding against any such parties for, any liability, loss, demands, claims, damages or causes of action, whether now known or unknown, for defamation, invasion of right to privacy, publicity or personality or any similar matter, or based upon or relating to the use and exploitation of my appearance, likeness and form.
